| Margie's Pets 896 Brown Road Hominy, OK 74035 We guarantee your puppy to be up to date on age appropriate vaccinations and de-worming at the time of delivery, and a record of such will be provided to you. Your puppy is guaranteed until one year of age against fatal and life altering congenital diseases which adversely affect the health of the animal. This includes severe hip dysphasia, heart, spine, kidney, pancreas, and liver problems of a severe life threatening or altering in nature. If the puppy dies within this one year time frame, you must pay to have the puppy autopsied to ascertain reason of death. If reason is congenital, we will replace the puppy. All items covered under this guarantee are by pet replacement only. A puppy showing any serious genetic or congenital defect will be replaced as follows: Animal must show the defect by one year of age. We must be notified of the problem within (2) business days of the licensed Veterinarian’s determination and have (1) report from an unassociated Veterinarian stating the same problem. We will then provide a replacement puppy of equal value. The puppy with the defect must be returned to us, or (euthanized with our permission) at buyers expense, with registration papers to qualify for replacement puppy. No replacement puppy will be given if the puppy has been euthanized without our permission. Buyer is responsible for transportation cost to and from us on a replacement puppy. This guarantee applies to the original party only and is nontransferable to a second party. Buyer’s Responsibilities You, as the buyer, are fully aware that owning a puppy means trips to the Veterinarian and understand that you will be responsible for all care and expenses for the puppy. Have your puppy examined by a licensed Veterinarian within 48 hours of delivery. We must be informed immediately of the examination’s result by e- mail. Failure to complete this crucial step will terminate the health guarantee on your puppy. This guarantee will then be null and void. If for any reason the buyer cannot keep the puppy sometime in the future, we ask that you contact us and we will try to find a home for him/her. By purchase of, or placing a deposit on the puppy, buyer agrees to the terms of this guarantee. This written agreement is a binding contract, entered into the State of Oklahoma and is the entire agreement between the buyer and the seller concerning the health conditions or loss of the puppy. Any litigation commenced shall take place in the State of Oklahoma in Osage County.
